Every required cue must be spoken aloud, in order. The split happens on these phrases, not on pauses — a section with no spoken cue is not recorded, and its answers get swallowed into the section before it.
Last night's miss: after a long gratitude run, the take went straight into the health answers with no "Health" — the tail landed in Step 10 as "unmapped answers." Say the cue, then pause a beat before the first answer.
"DBT" and "Health" each open their own card. No cue, no card.
Step 10 needs the words "nightly inventory" — "10th step" alone will not split.
Don't say "10th step" early as a preview — first mention steals the boundary.
Good/Funny need the plural: "good things", not "a good thing."
Meeting needs "log a meeting" / "meeting log" — "meeting" alone won't do it.

Notes / triggers / events — or say "no notes"
20% and 15 minutes are working lines, not rules. Experts commonly use "off task 20%" and "a break over 15 minutes" (a normal job allows two 15-minute breaks plus lunch), but they vary — confirm with SMRLS before a filing relies on them.
No numbers. Date is never guessed from speech — set separately.
